I was informed this was a scam. I gave them my email address and cc #. The card was locked immediately. I installed a fraud alert with Equifax. Notified the bank. Police report is pending. Password to my email account was changed.

Scam for credit card information

Costco survey is completed and you then are provided with rewards that require you to cover S & H fee.
Standard address entry, and then credit card info. you enter first card and it is declined, then you enter a second card and it is declined. Now they have your information. Contacted Costco who said it is not their survey and is most likely a scam.
Can forward email.
